A structured interview was conducted with the Claimant to gain a contextual understanding of the circumstances surrounding the lead-up to the incident, the incident itself and the post incident impact on the Claimant.

A full report on the incident based on disclosures, statements and interviews was produced.  This report was commissioned by counsel on behalf of the Court and submitted in accordance with Part 35 of the Civil Procedure Rules and the associated Practice Direction.  The report, presented in a standard and compliant format, addressed operational and administrative elements of the incident, highlighting areas of responsibility and authority.
